Membership entitles one person to agreed upon chiropractic care during regular business hours. Memberships can not be shared. Family members can be added for $10/month per person. Early termination penalties will apply. Membership is based on a 48-week calendar allowing for personal time away from the office. 15% off subscription if paid up front.

Any changes to the schedule will be sent out to active patients via email and/or posted on Connect Chiropractic's Facebook page. 

Must have a card on file unless paying full subscription. ​Preferred method of payment is cash, but checks are also accepted. A 3% processing fee is added to all credit and debit card payments, except for monthly auto payments.
